Case Management

 

Good case management is the practical side of rehabilitating a patient and ensures all the practical aspects of a well-designed, personalised treatment plan come together in a co-ordinated way for maximum success.

 

Often the family of a brain injured patient get swept up in the multitude of complex issues that a litigation case can produce, and managing all the tiny details of their loved ones rehabilitation becomes overwhelming. They may not have access to or knowledge of where to find the best help and support and this ultimately impacts on the rehabilitation process.

 

Using my network of contacts in the health care and social service systems, I'll make sure a patient and their family get access to the best quality social, vocational, educational and clinical services possible. This includes:

 

  1. Finding adapted housing or ensuring existing housing is altered appropriately
  2. Identifying and claiming all relevant monetary benefits
  3. Accessing appropriate statutory services
  4. Recruiting, training and supervising a multidisciplinary team of key people involved in a patient's recovery, such as neuro-occupational therapists and patient 'buddies'
  5. Liaising with medical professionals, such as GPs, neurologists and psychiatrists, throughout the process, and ensuring the right ones are brought on board at the right time.
  6. Setting up and monitoring care regimes to ensure rehabilitation goals are met
